These Terms and Conditions ("Terms") govern your use of the Jalaram Laboratory & Diagnostic Centre website (the "Website") and the diagnostic, pathology, radiology, microbiology, and cardiac investigation services we provide ("Services"), whether booked online, on WhatsApp, by phone, or in person at our facility at Shishudip Hospital, Station Road, Bardoli, 394601, Gujarat.
By browsing the Website, booking a test, or visiting our centre, you agree to these Terms. If you do not agree with them, please do not use the Website or avail our Services.
1. Definitions
- "We," "us," "our," or "the Company" refers to Jalaram Laboratory & Diagnostic Centre.
- "You" or "Customer" refers to anyone who visits the Website, books a test, or uses our Services.
- "Services" means any diagnostic test, health package, home sample collection, or related service we offer.
2. Eligibility
Our Services are open to individuals of any age, though anyone under 18 must be accompanied or represented by a parent or legal guardian who consents to the test and provides any required information on their behalf. By booking a test for yourself or a family member, you confirm that you have the authority to do so.
3. Our Services
We offer diagnostic testing across pathology, radiology, microbiology, biochemistry, and cardiac investigations, along with preventive health packages and home sample collection. The tests, packages, and prices listed on the Website are indicative and may change from time to time; we will confirm the applicable price and preparation instructions at the time of booking.
We may add, modify, or discontinue any test, package, or service at our discretion, and we will let you know if this affects a booking you've already made.
4. Booking and Home Sample Collection
- You can book a test by walking in, calling us, or messaging us on WhatsApp; a doctor's prescription is not required for most routine tests, though we recommend one for hormone-related or specialised tests.
- For home sample collection, please provide an accurate address, a reachable phone number, and be available at the scheduled time. We may reschedule a visit if our team cannot reach you or access the location.
- Please carry a valid photo ID (and any prescription, if applicable) when visiting our centre or when our staff arrive for home collection.
- Some tests require fasting or other preparation. It's your responsibility to follow the instructions we share at the time of booking, as not doing so can affect your results and may require a repeat sample.
5. Test Reports and Their Use
We prepare test reports based on the sample provided and the information you share with us. Our reports are intended to support your treating doctor's diagnosis and are not a substitute for professional medical advice, diagnosis, or treatment. Please always discuss your results with a qualified doctor before making any medical decision based on them.
Reports are usually delivered digitally, via WhatsApp or email, within the turnaround time communicated at booking; some specialised tests may take longer. We are not responsible for delays caused by courier services, network issues, or circumstances beyond our reasonable control, though we will do our best to keep you informed.
It's your responsibility to give us accurate personal and medical information at the time of booking. We are not liable for any inaccuracy in a report that results from incomplete or incorrect information provided by you.
6. Pricing, Payment and Offers
Prices for tests and health packages are as displayed on the Website or as quoted to you at the time of booking, and are subject to change without prior notice. Payment is generally collected at the time of sample collection or report delivery, in cash, by card, or through UPI, unless a different arrangement is agreed with you in advance.
Discounted packages and promotional offers are valid for the period mentioned and may be withdrawn or modified at our discretion. Offers cannot usually be combined unless stated otherwise.
7. Cancellation and Rescheduling
You may cancel or reschedule a booking by contacting us on WhatsApp or phone before your sample is collected, at no charge. Once a sample has been collected and processing has begun, we are unable to offer a refund, since laboratory costs are incurred at that point. If we are unable to process a sample due to a quality issue on our end, we will arrange a free re-collection wherever possible.
8. Acceptable Use of the Website
When using the Website, you agree not to:
- use it for any unlawful purpose or in a way that could damage, disable, or impair it
- attempt to gain unauthorised access to our systems or another user's information
- copy, scrape, or reproduce content from the Website for purposes other than your personal, non-commercial use
- submit false, misleading, or fraudulent information when booking a test
9. Intellectual Property
All content on the Website, including our logo, text, images, and blog articles, belongs to Jalaram Laboratory & Diagnostic Centre unless otherwise credited, and may not be copied, reproduced, or used elsewhere without our written permission.
10. Third-Party Links and WhatsApp Communication
The Website may link to third-party sites, and we use WhatsApp Business to help you book appointments and receive reports. We aren't responsible for the content, privacy practices, or availability of third-party services, including WhatsApp, which are governed by their own terms.
11. Disclaimer of Warranties
The Website and our Services are provided on an "as is" basis. While we take care to maintain accuracy and reliability in our testing processes, we do not guarantee that the Website will always be available, error-free, or uninterrupted, or that every test result will be free of the natural variability inherent to laboratory diagnostics.
12. Limitation of Liability
To the extent permitted by law, Jalaram Laboratory & Diagnostic Centre will not be liable for any indirect, incidental, or consequential loss arising from your use of the Website or our Services, including delays, technical issues, or decisions made based on a report without consulting a doctor. Nothing in these Terms limits our responsibility for death or personal injury caused by our proven negligence, or any other liability that cannot be excluded under Indian law.

You agree to indemnify us against any claim or loss arising from your misuse of the Website, a breach of these Terms, or false information you provide to us.
